A questionnaire is a customer survey consisting of multiple questions with answer options shown in a separate page of event edit form. Usually, used at a meeting where the customer can quickly mark answers providing useful feedback:

Click New Questionnaire to Creating or Editing a Questionnaire.
To edit a questionnaire, right click it and select Edit.
